Sildenafil and Food, Alcohol and Timing: A Practical Guide

Sildenafil works well for many men with erectile dysfunction, but small everyday details can change how well it works on a given night. A heavy dinner, a few too many drinks or poor timing are the most common reasons men say "it didn't work this time". This guide covers the practical side: when to take it, what to eat, how alcohol fits in, and a few things to avoid altogether.

Timing: the one-hour rule

Sildenafil is usually taken about one hour before sex. For many men it starts to work within 30 to 60 minutes, and the useful window generally lasts around four hours. That window is not an erection lasting four hours; it is the period in which arousal is more likely to lead to an erection.

A few timing tips:

  • Take it no more than once in 24 hours, whatever the strength.

  • Don't take a second tablet because the first seems slow. Give it time.

  • Remember that sexual stimulation is still needed. Sildenafil does not work on its own.

Food: why a big meal slows it down

Sildenafil is absorbed best on an empty stomach or after a light meal. After a large or high-fat meal (a burger and chips, a creamy curry, a full cooked breakfast), absorption slows. The tablet can take up to an hour longer to start working, and the peak effect may be a little weaker.

Practical options:

  • Take the tablet before the meal rather than straight after it.

  • Keep the meal lighter if you want it to work that evening.

  • If you have already eaten a heavy meal, allow extra time rather than taking more.

Food does not make sildenafil unsafe. It simply shifts the timing.

Grapefruit

Grapefruit and grapefruit juice slow the breakdown of sildenafil in the body, which can raise its level in the blood. That can make side effects such as headache, flushing and dizziness more likely. It is sensible to avoid large amounts of grapefruit juice around the time you take a dose.

Alcohol: a little versus a lot

A drink or two is generally fine for most men taking sildenafil. Heavier drinking is a different story:

  • Alcohol itself makes erections harder to get and keep, so it works against the medicine.

  • Both alcohol and sildenafil can lower blood pressure, so together they can cause dizziness, light-headedness or fainting, especially when standing up quickly.

  • Heavy drinking can make headache and flushing worse.

If sildenafil seems not to work after a night of heavy drinking, the alcohol is the more likely cause than the tablet.

Things never to mix

Some combinations are dangerous, not just unhelpful:

  • Nitrates, including glyceryl trinitrate sprays and isosorbide tablets for angina

  • "Poppers" (amyl or alkyl nitrites)

  • Riociguat, used for some forms of high blood pressure in the lungs

Tell your prescriber about every medicine you take, including alpha-blockers, some HIV and antifungal medicines and certain antibiotics, as several of these can raise sildenafil levels.

A sensible routine

  1. Eat a light meal, or take the tablet before eating.

  2. Take your prescribed dose about an hour before you expect to have sex.

  3. Keep alcohol moderate.

  4. Allow time for arousal and don't rush to judge the result.

  5. Try it on several separate occasions before deciding it doesn't work, then talk to your prescriber.

Doses and ordering

Sildenafil is licensed at 25 mg, 50 mg and 100 mg. Strengths of 150 mg and 200 mg are sometimes sold, but they are not licensed doses; only take what your prescriber has set.

If you decide to buy generic sildenafil online, a valid prescription is required for every order. Our guide on how to buy sildenafil online explains the brands, strengths and ordering steps, and our article on how long sildenafil lasts looks more closely at the timing window. Whenever you buy medicine online, stick to the dose on your prescription.

This is general information, not medical advice. Speak to your doctor or pharmacist before starting any medicine.
